Could Your Medications Be Making You Gain Weight?
You’re eating well. You’re exercising. You’re trying to stay active and make good choices — but the scale keeps creeping up.
One factor that is often overlooked is your medication list.
Several commonly prescribed medications can contribute to weight gain or make weight loss more difficult. This doesn’t mean you should stop a medication that you need. It does mean that if your weight has changed unexpectedly, reviewing your medications with your healthcare provider may be an important part of the conversation.
How Can a Medication Cause Weight Gain?
Medication-related weight gain can happen through several different mechanisms.
Some medications can increase appetite or food cravings, making it easier to consume more calories without realizing it.
Others may affect insulin sensitivity, glucose regulation, or fat storage.
Some medications cause fluid retention, which can increase the number on the scale even when body fat has not significantly changed.
Others can cause fatigue, sedation, or decreased exercise tolerance, indirectly reducing daily energy expenditure.
And in some cases, medications can influence the brain pathways involved in hunger, fullness, and reward.
The result? A medication that is helping one health condition may simultaneously be making weight management more difficult.
1. Antidepressants
Weight effects vary considerably between antidepressants.
Some antidepressants are more frequently associated with weight gain during long-term treatment, particularly certain SSRIs and older antidepressants.
Examples that may be associated with weight gain include:
Paroxetine (Paxil)
Mirtazapine (Remeron)
Some tricyclic antidepressants such as amitriptyline
Other antidepressants tend to have a more weight-neutral profile.
Bupropion (Wellbutrin) is particularly interesting because it is generally associated with less weight gain than many other antidepressants and may produce modest weight loss in some patients. Bupropion is also one component of the FDA-approved weight-management medication naltrexone/bupropion.
The best antidepressant, however, is not determined by weight alone. Anxiety, depression severity, sleep, other medications, medical history, and side effects all need to be considered.
2. Antipsychotic Medications
Certain atypical antipsychotic medications can have significant metabolic effects.
Medications such as olanzapine and clozapine are particularly well known for their potential to increase appetite and body weight and negatively affect glucose and lipid metabolism.
Other agents may have less metabolic impact.
Patients taking these medications may benefit from monitoring:
Weight and waist circumference
Blood pressure
Fasting glucose or HbA1c
Lipid levels
These medications can be extremely important for mental health, so they should never be discontinued solely because of concerns about weight without discussing the risks and alternatives with the prescribing clinician.
3. Beta Blockers
Beta blockers are commonly prescribed for high blood pressure, heart conditions, migraines, tremor, and other conditions.
Older beta blockers, including metoprolol and atenolol, have been associated with modest weight gain in some patients.
Possible explanations include decreased energy expenditure, fatigue, and reduced exercise tolerance.
Not every beta blocker has the same metabolic profile, and not every patient will experience weight gain. If weight or fatigue became an issue after starting a beta blocker, it may be reasonable to ask whether another medication would appropriately treat the underlying condition.
4. Insulin and Certain Diabetes Medications
This one can seem counterintuitive: a medication that improves blood sugar can sometimes increase weight.
Insulin can promote weight gain because it allows glucose to move from the bloodstream into cells and decreases the loss of calories through the urine when diabetes becomes better controlled.
Certain medications that stimulate insulin release, particularly sulfonylureas, can also contribute to weight gain.
Fortunately, modern diabetes treatment offers several medications with more favorable weight profiles.
GLP-1–based therapies, including semaglutide and tirzepatide, can produce substantial weight loss in appropriate patients.
SGLT2 inhibitors, such as empagliflozin, are also generally associated with modest weight reduction rather than weight gain.
The right treatment depends on much more than weight, including kidney function, cardiovascular risk, glucose levels, cost, and medical history.
5. Corticosteroids
Long-term systemic corticosteroid treatment, such as prednisone, can significantly affect body composition and metabolism.
Chronic exposure can contribute to:
Increased appetite
Insulin resistance
Elevated blood glucose
Fluid retention
Changes in fat distribution
Loss of muscle mass
The dose and duration of treatment matter tremendously. A short steroid course is very different from months of systemic corticosteroid therapy.
When long-term steroids are medically necessary, monitoring metabolic health and body composition becomes especially important.
6. Anti-Seizure and Nerve-Pain Medications
Some medications used for seizures and neuropathic pain can contribute to weight gain.
Examples include:
Gabapentin
Pregabalin
Valproate
Weight effects vary considerably between medications, and some alternatives may actually be relatively weight neutral or associated with weight loss.
Again, the goal isn’t simply to replace a medication because it can cause weight gain. The goal is to determine whether the medication remains the best choice for that particular patient.
7. Antihistamines
This category surprises many people.
Some antihistamines — particularly medications with stronger sedating effects — have been associated with increased appetite and weight gain.
One proposed explanation involves blocking histamine signaling in the brain, which participates in appetite regulation.
If someone is using an antihistamine every day rather than occasionally, it may be worth discussing the long-term treatment strategy with their healthcare provider.
Weight Gain Is More Complicated Than Calories In vs. Calories Out
Energy balance still matters, but human metabolism is far more complicated than simply telling someone to “eat less and exercise more.”
Medications can influence:
Hunger → cravings → insulin sensitivity → glucose metabolism → energy expenditure → fluid balance → fatigue → physical activity → body composition.
That is why unexplained weight gain deserves a comprehensive evaluation.
In addition to reviewing medications, a clinician may consider thyroid function, glucose and insulin regulation, sleep, hormonal changes, nutrition, physical activity, alcohol intake, stress, and other medical conditions.
Don’t Stop Your Medication Because of the Scale
This is the most important point.
Do not abruptly stop a prescription medication because you believe it is causing weight gain.
Some medications can cause serious withdrawal symptoms or worsening of the underlying condition when suddenly discontinued.
Instead, ask your healthcare provider:
“Could any of my medications be contributing to my weight gain, and is there an equally effective alternative with a more favorable metabolic profile?”
Sometimes the answer will be yes.
Sometimes the medication is still clearly the best choice.
And sometimes the solution isn’t changing the medication at all — it’s developing a strategy to manage its metabolic effects.
The Bigger Picture: Medication Optimization
